Sports Physicals

Schedule and get your REQUIRED physical ASAP

SJH & SHS Student-athletes, SHS Marching Band, SHS Cheer, and SJH & SHS Special Olympians must get a physical if they:

  • are new to the program

  • are an incoming 7th, 9th, or 11th grader

  • have a pre-existing medical condition

  • were injured since their last physical and required a doctor's release

  • Student trainers are also required to obtain one physical while in high school.

  • If you have one from your previous school that is only one year old, please email a copy to athletics@snyderisd.net.

You will not be able to participate in summer strength and conditioning programs or athletics unless you are enrolled and registered with Snyder ISD for the upcoming school year and have a current physical on file.

OPTION 1 - ON-SITE SPORTS PHYSICAL CLINIC

Attend the on-site sports physical clinic with Cogdell Memorial Hospital (Physicals are FREE through this event)

  • Snyder Junior High Students: May 12th 6pm - 8pm (@ SHS Indoor Facility)

  • Snyder High School Students: May 21st 6pm - 8pm (@SHS Indoor Facility)

OPTION 2 - SCHEDULE AN APPOINTMENT

Schedule an appointment at Cogdell or with your Primary Care Provider.

  • Physicals are FREE at Cogdell from May 1st through July 31st. Please call 325-573-1300 to schedule an appointment. After July 31st, physicals at Cogdell are $25.

IMPORTANT STEPS

  1. Download and complete the Medical History Form BEFORE your appointment

    Hard copies are also available in the Athletic Office or at Cogdell Family Clinic.
